29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) 16733 Promenade St. Broomfield, CO 80023: On or about June 15, 2024, and at times prior, employees were exposed to fall hazards of approximately 21 feet while conducting framing activities on the top plate of the second level without utilizing personal fall protection or guardrails. (b) 16733 Promenade St. Broomfield, CO 80023: On or about June 15, 2024, and at times prior, employees were exposed to fall hazards of approximately 10 feet while conducting framing activities without utilizing personal fall protection or guardrails.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) 16733 Promenade St. Broomfield, CO 80023: On or about June 15, 2024, and at times prior, employees were exposed to falls of approximately 21 feet to the ground while conducting framing activities on a residential building. The employer did not provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work. Employees worked on the top plate of the structure without personal fall protection or guardrails.
