A) Riva Concrete, Inc. 657 Morris Turnpike, Springfield NJ 07081 - On June 18, 2024, the employer did not ensure employees were wearing Personal Protective Equipment (PPE), such as hard hats and safety glasses while working in an excavation 11'8" in depth below grade exposing workers to possible eye and head injuries.

29 CFR 1926.652(a)(1): Each employee in an excavation were not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. Riva Concrete, Inc. - 657 Morris Turnpike, Springfield NJ 07081 - On June 18, 2024, employees were in the process of placing rebar, hand digging and setting up formwork for concrete placement in an excavation that was approx. 11' 8" in depth x 64' L x 36' wide . The employees were exposed to cave-in/engulfment hazard while working in an unprotected excavation. No protective system in place (shoring, shielding, benching, or sloping) was provided for employee protection.
