29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Slitting Department:  On, or about, June 11, 2024, an employee was seriously injured when his arms were pulled under the lay-down bar on Slitter #13.  The operation of the jog and emergency stop controls located on the rear of the machine had been modified/disabled due to the machine being set in "stop mode" instead of "run mode" without the knowledge of the employee.
