Colonial Metal Products, Inc. 2350 Quality Lane West Middlesex, PA 16159: On or about 05/23/24 and at times prior thereto, Employees are exposed to hazards associated with the Hyster forklift (Serial #D004D01701T) forks falling off the carriage while being adjusted by the operator. b.) Colonial Metal Products, Inc. 2350 Quality Lane West Middlesex, PA 16159, Shipping/Receiving Area: On or about 06/20/24 and at times prior thereto, the brake pedal on the Hyster forklift (Serial #D004D01701T) was missing the rubber grip/anti-slip texture on the face of the brake pedal.

29 CFR 1910.178(l)(3)(i)(M): Powered industrial truck operators shall receive initial training in the following topics to include but not limited to operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate: a.) Colonial Metal Products, Inc. 2350 Quality Lane West Middlesex, PA 16159: On or about 06/20/24 and at times prior thereto, the powered industrial truck training did not cover hazards identified in the Hyster #6 Owner's Manual. The hazards not covered in the training include, but is not limited to, hazards associated with fork removal and adjustment.
