29 CFR  1926.1408(a)(2): Determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line. If so, the employer must meet the requirements in Option (1), Option (2), or Option (3) of this section.  On or about June 19, 2024, with respect to a worksite located at 2401 Velp Ave, Green Bay, WI 54303, the employer did not determine whether the load, load line or parts of the Multi-use Boom Truck used during sign removal activities could get closer than 20 feet to a power line and institute one of the following protective measures: 1) De-energize and ground, 2) Maintain 20 Foot Clearance, 3) Maintain clearances identified in Table A of this standard.

29 CFR  1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person must be maintained. If that ability is interrupted at any time, the operator must safely stop operations requiring signals until it is re-established and a proper signal is given and understood.  On or about June 19, 2024, with respect to a worksite located at 2401 Velp Ave, Green Bay, WI 54303, the employer did not ensure that the ability to transmit signals between the operator and signal person were maintained while moving the load line and boom of a Multi-use Boom Truck into an encroachment area of adjacent power distribution lines.

29 CFR  1926.1408(g)(1): Power line safety (up to 350 kV)--equipment operations. The employer must train each operator and crew member assigned to work with the equipment on all of the requirements identified in standards 1926.1408(g)(1)(i)-1926.1408(g)(1)(v) of this standard.  On or about June 19, 2024, with respect to a worksite located at 2401 Velp Ave, Green Bay, WI 54303, the employer had not ensured that each employee working with equipment near power distribution lines had been trained in the recognition of hazards working near power lines, methods to avoid contact with powerlines and procedures following accidental contact with powerlines.

On or about June 19, 2024, with respect to a worksite located at 2401 Velp Ave, Green Bay, WI 54303, the employer did not ensure that an employee operating a Multi-use Boom Truck crane has been trained, certified/ licensed, and evaluated prior to allowing the employee to operate the boom crane.
