Location: a) Worksite; formwork; shear wall supporting 2nd floor a) On or about 6/19/2024, an employee performing formwork fell approximately 8 feet from the shear wall. The employer did not ensure that employees used fall protection, such as personal fall arrest systems, safety net systems, or positioning device systems, at all times when performing formwork.
