29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to a specific statement of the intended use of the procedure, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, and specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. The Extruder machine 31: On or about June 10, 2024, and at times prior, the employer exposed employees to struck-by hazards from the moving parts of the Sytec Extrusion, Equipment ID: SX31 machine, in that lockout procedures did not contain a specific statement of the intended use of the procedure, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, and specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, while employees are entering the machine area for maintenance.
