29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about 6/11/2024, at a work-site located at 501 Rockside Rd, Seven Hills, Ohio 44131; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 9.1 feet.

29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 6/11/2024, at a work-site located at 501 Rockside Rd, Seven Hills, Ohio 44131; the employer did not ensure employees working on the roof of a new construction residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns and hand tools.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about 6/11/2024, at a work-site located at 501 Rockside Rd, Seven Hills, Ohio 44131; the employer did not ensure employees working on a roof of a commercial facility were protected from a fall of approximately 9.1 feet by utilizing a fall protection system.  Dira Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Fall Protection), which was contained in OSHA inspection number 1729199, citation number 1, item number 1a and was affirmed as a final order on (3/28/2024), with respect to a workplace located at 909 Wilbur St, Barberton Ohio 44203.
