29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use.  In the Alternative: 29 CFR 1926.103: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.134 of this chapter.  On or about June 13, 2024, and at times prior thereto, employees spraying foam insulation were required to wear a full-face respirator. The employer did not implement a written respiratory protection program to include but not limited to the following: a) Ensuring employees were medically cleared to wear a respirator. b) Ensuring employees were fit tested on respirator worn.  c) Training employees on the program.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   In the Alternative: 29 CFR  1926.59: The requirements applicable to construction work under this section are identical to those set forth at � 1910.1200 of this chapter.  On or about June 13, 2024, and at times prior thereto, employees were using chemicals without being trained on the hazards of the chemicals.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.  On or about June 13, 2024, and at times prior thereto, employees were using a scissor lift at heights of approximately 10 feet without the use of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 13, 2024, and at times prior thereto, employees were exposed to fall hazards above 6 ft and the employer did not developed a fall protection training program.
