On or about June 21, 2024, at the above addressed jobsite, an employee using a portable electric hand saw was not utilizing eye protection, thereby exposing the employee to hazards associated with eye injuries.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about June 21, 2024, the portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.

On or about June 21, 2024, at the above addressed jobsite, an employee working on the ground level, under a worker performing residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.  Jorge Munoz dba Jorge Munoz was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.100(a), which was contained in OSHA Inspection number 1529411, citation number 1, item 1 and was affirmed as a final order on June 22, 2021, with respect to a workplace located at 24449 103rd St., Naperville, Illinois.

On or about June 21, 2024, at the above addressed jobsite, employees performing residential roofing operations, eight (8) feet four (4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Jorge Munoz dba Jorge Munoz was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection number 1529411, citation number 1, item 2 and was affirmed as a final order on June 22, 2021, with respect to a workplace located at 24449 103rd St., Naperville, Illinois.
