29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  a) 1821 Center Street Lebanon, PA - On or about June 24, 2024, the employer did not have a competent person on the job site, and none of the employees who worked at the site inspected the job site, identified predictable fall hazards, or took immediate actions to correct the hazards. As a result, employees worked from improperly positioned ladders. They performed roofing activities at approximately 20 feet without implementing fall protection, exposing the employees to the hazard of falling to grade.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 1821 Center Street Lebanon, PA - On or about June 24, 2024, employees were performing residential roofing work, approximately 20 feet above grade, without utilizing any means of fall protection.   Corrected during the inspection, no abatement certification or documentation required.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  a) 1821 Center Street Lebanon, PA - On or about June 24, 2024, employees used a ladder to access the roof, 20 feet above grade.  The ladder was not used at an angle that permitted adequate support of the ladder exposing employees to a fall hazard.    Corrected during the inspection, no abatement certification or documentation required.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) 1821 Center Street Lebanon, PA - On or about June 24, 2024, employees utilized an extension ladder to access and egress the second story roof, approximately 20 feet to the second story roof, while carrying 8ft x 4ft sheets of plywood that could cause the employee to lose their balance and fall.  Corrected during the inspection, no abatement certification or documentation required.
