29 CFR 1910.22(b): The employer did not ensure that each walking-working surface can support the maximum intended load for that surface. On or about July 24, 2024, on the southwest side of the site, an employee was disconnecting the ground wire from the catwalk/staircase, which was not properly evaluated to ensure that the walking-working surface could support the maximum intended weight. The lack of evaluation contributed to the collapse of the walking-working surface, which injured an employee.

On or about July 24, 2024, the employer did not instruct each employee in recognizing, avoiding, controlling, or eliminating unsafe conditions and the regulations applicable to their environment. A qualified person did not train the Employee to assess and evaluate the walking-working surface to support the maximum intended load for that surface.
