OSH ACT of 1970 Section (5)(a)(1) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a) On June 17, 2024, the employer failed to ensure that adequate first aid and medical treatment were provided to an employee experiencing signs and symptoms of a heat-related illness. Specifically, when the employer was informed at 5:19 p.m. that an employee was experiencing a potentially serious illness, emergency medical care was not provided to the location of the employee.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a) On or about April 15, 2024 for case 2894-85, the employer did not complete column (F) on the OSHA 300 Log with a complete description, in that the resulting injury to the right knee was not described. b) On or about May 8, 2024 for case 2963-37, the employer did not complete column (F) on the OSHA 300 Log with a complete description, in that the resulting injury to the right lower leg was not described. c) On or about June 17, 2024 for case 3078-77, the employer did not complete column (F) on the OSHA 300 Log with a complete description, in that the type of heat-related illness and how the illness occurred was not described.
