On or about 05/29/2024, the employer did not ensure that employees were not using the top step of the metal 24"step ladder as a step to access the feeder section of the 201 Flexo Line located 56" from the ground.

29 CFR  1910.28(b)(15): Walking-working surfaces not otherwise addressed.  Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by:   (i)Guardrail systems;  (ii)Safety net systems; or  (iii)Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.  a.) On or about 05/29/2024 the employer did not ensue that employees working on the 201 Flexo Line feeder machine had fall protection in the form of either a guardrail, safety net system, or personal fall protection in that employees were climbing 56" from the floor to the the top of the feeder machine when removing jammed cardboard in the feeder machine.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (2) Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.  (3) Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  (4) Lockout or tagout device application.  (4)(i) Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  (4)(ii) Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position.  (5) Stored energy.  (5)(i) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.  (5)(ii) If there is a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation shall be continued until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists.  (6) Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished.  b.) On or about September 10, 2024, at the 208 Line, the authorized employee cleaning under the 208 Stacker machine were exposed to a caught in hazard from the moving conveyor belts on the stackers due to not applying the following required elements of  d2, d3, d4 and d6 of the established lockout procedure: 1. the energy sources for the moving conveyor was not turned off prior to cleaning under the moving conveyor; 2. the energy sources such as electrical and pneumatic were not isolated prior to cleaning; 3. no locks or id tags were placed on each of the equipment's energy sources by each employee performing the cleaning; 4. no restart or other testing was performed to verify that the energy sources were isolated.  Employees were exposed to caught in hazards.
