29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  On or about 6/25/2024, at a work-site located at 1429 Stone Crossing ST NE, Canton Ohio 44721 the employer did not ensure that a ladder used for accessing the roof of a residential structure extended at least 3 feet beyond the upper landing surface and/or was secured at the top with a grasping device, exposing employees to falls over 10.8 feet.  Miller Brothers Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1603558, citation number 1, item number 01 and was affirmed as a final order on (8/16/2022), with respect to a workplace located at 8990 Elsmere DR, Parma Ohio 44130.  Miller Brothers Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1413661, citation number 01, item number 02 and was affirmed as a final order on (10/21/2019), with respect to a workplace located at 6007 Highpoint DR, Medina Ohio 44256.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about June 25, 2024, at a work location located at 1429 Stone Crossing ST NE, Canton Ohio 44721 the employer did not ensure that each employee engaged installing roofing material with air driven nail guns and hand tools were protected from eye and or face injury by utilizing personal protective equipment.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  On or about June 25, 2024, during the installation of roofing material at a job site located at 1429 Stone Crossing ST NE, Canton Ohio 44721; the employer did not ensure that all employees working on the roof of a single family home were protected from a fall of approximately 10.8 feet by utilizing a fall protection system.
