29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). At the jobsite located at the intersection of Amour & Wyandotte, Kansas City, MO 64111: Employees were exposed to struck-by and engulfment hazards while performing work in an excavation with a depth exceeding 5' without any means of cave in protection. The Employees were utilizing shovels to hand dig around a water pipe that was being removed.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. At the jobsite located at the intersection of Amour & Wyandotte, Kansas City, MO 64111: Employees were exposed to fall hazards while performing work in an excavation. The employees were utilizing an extension ladder as a means of egress out of the excavation. The ladder did not extend three feet past the upper landing surface being accessed.
