29 CFR 1910.305(g)(1)(iii):If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet. a) MAINTENANCE SHOPS: A 14AWG 3-conductor flexible cord, used as a tethered single receptacle, was wired permanently into a wall mounted junction box, exposing employees to fire and electrical hazards. b) FRONT ENTRY GATE: A 14AWG 3-conductor flexible cord, used as a tethered single receptacle, was wired permanently into a wall mounted junction box, exposing employees to fire and electrical hazards.
