29 CFR 1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a. 1 St Mary's Pl, Denville, NJ - Denville Township Municipal building The employer did not ensure that employees were protected from fall hazards. Employees engaged in the servicing of a rooftop HVAC worked in areas where the employee were exposed to fall hazards. Following the ignition of an acetylene bottle, the employee was able to walk at or near an unprotected edge and was not protected from falls by the use of a personal fall arrest system or a guardrail system. Violation occurred on or about June 18, 2024

29 CFR 1910.253(c)(4)(viii): Portable outlet headers were not provided with frames which would support the equipment securely in the correct operating position and protect them from damage during handling and operation: a. 1 St Mary's Pl, Denville, NJ - Denville Township Municipal building - Roof The employer did not provide a frame to support acetylene tanks when used at jobsites. Employees engaged in the servicing/repairs of a roof HVAC unit used a torch for pipe sweating which was not affixed to a cart or other frame in which to secure the cylinder from damage during handling or operation. Violation occurred on or about June 18, 2024
