29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  On or about June 26th, 2024, employees engaged in roofing operations located at 20901 Larimore Ave, Elkhorn, Nebraska 68022 were exposed to struck-by hazards in that employees were observed using pneumatic nail guns without eye protection while installing roof shingles.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 26th, 2024, employees engaged in roofing operations located at 20901 Larimore Ave, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer failed to provide employees fall protection training on the hazards related to working at elevated heights prior to the employees conducting residential roofing at heights in excess of 6 feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 26th, 2024, employees engaged in roofing operations located at 20901 Larimore Ave, Elkhorn, Nebraska 68022 were exposed to fall hazards in that an employee used an extension ladder as means of access and egress on and off a residential roof without having extended the ladder at least 3ft above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    On or about June 26, 2024, employees performing residential roofing operations were exposed to fall hazards on the jobsite located at 20901 Larimore Ave, Elkhorn, NE 68022, in that the employees were were observed installing roof shingles at heights greater than 6ft without any form of fall protection.    Urbina Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1614037, citation number 1, item number 1and was affirmed as a final order on 9-19-2022, with respect to a workplace located at 5209 Orchard Ave, Bellevue, NE.    Urbina Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1611220, citation number 1, item number 1 and was affirmed as a final order on 10-20-2022, with respect to a workplace located at 7506 Grover Street, Omaha, NE.
