Enterprises LTD. On or about June 26, 2024 at the address of 2045 Lakota Dr. Findlay OH 48540: The employer did not ensure that the scaffold platform (Job box) used to hoist employees to higher levels was attached to the forks of the Pettibone B68 telehandler forklift while the platform was occupied. Employees using scaffold in this condition as well as others working in and around forklift operating areas are exposed to struck-by injuries, fall injuries, tipping, and overturning hazards.

Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated a) V.O. Enterprises LTD. On or about June 26, 2024 at the address of 2045 Lakota Dr. Findlay OH 48540: The employer did not ensure that aerial lift operator donned and secured their personal fall arrest systems before operating telescopic boom lift (Aerial Lift). Employees utilizing aerial lift to performed steel erection work activities (installing prefabricated metal building) were exposed to fall hazards from approximately 19 feet or greater to the ground below.
