29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Location: 2707 Cranberry Highway Wareham MA On or about June 26, 2024, Employees were exposed to hazards associated to component failure of PITs due to modification to lifting components without prior written approval of manufactures. a) Yale PIT Serial number L87263348 has holes cut into the forks to accommodate retaining pins for a locally made lifting mechanism. b) JLG Telehandler Model G12-55A serial number 0160060493 has had portions of the underside of the boom cut away to accommodate metal plates that serve to reinforce the boom due to potential cracking.

29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Location: 2707 Cranberry Highway Wareham MA On or about June 26, 2024, employees are exposed to rollover and tipping hazards associated with the use of a job made lifting attachment that is not approved by the manufacture of the PIT.
