29 CFR  1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling:  a) Jobsite: 730 Montrose St, Philadelphia, PA - On or about June 26, 2024, the employer did not ensure that employees conducting siding activities utilizing a pump jack scaffold, approximately 25 feet above the ground level, was within 14 inches of the working surface. Employees were working from a scaffold approximately 17 inches from the working surface, thereby exposing them to fall hazards.

29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite: 730 Montrose Street, Philadelphia PA - On or about June 26, 2024, the employer did not ensure each employee was protected from falls by the use of personal fall arrest systems or guardrail systems. Employees were conducting masonry activities from a pump jack scaffold without a fall protection system or guardrail system, thereby exposing employees to fall hazards of approximately 25 feet.

29 CFR  1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled:  a) Jobsite: 730 Montrose St, Philadelphia, PA - On or about June 26, 2024, the employer did not ensure poles at the bottom of a pump jack scaffold were secured to the structure by rigid triangular bracing. Employees were performing siding activities from an unsecured pump jack scaffold approximately 25 feet from the ground, thereby exposing them to fall hazards.
