29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: On or about September 5, 2024, it was documented employees engaged food service activities in the kitchen of Glazed, LLC located at 1726 North 120th Street, Omaha, NE were exposed to slip and fall hazards in that the employer did not ensure the kitchen and lobby floor were clean and free from grease, oil, and/or other slippery substance(s).

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting: On or about June 26, 2024, it was documented that employees engaged in food preparation and service at Glazed, LLC doing business as Hurts Donut Co. located at 1726 North 120th Street, Omaha, Nebraska were exposed to fire hazards in that the employer provided at least one fire extinguisher in the establishment but did not educate and familiarize employees on fire extinguisher use and associated hazards.
