29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment. a) On or about June 26, 2024, the employer did not ensure unused openings in the Cutler-Hammer UNITROL Motor Control Center (Serial Number - 6AFI386853-A), located in the CRANE MCC room, were effectively closed exposing employees to a 440-volt electrical shock hazard when locking out skimmer buckets, pulling fuses, switching disconnects and maintaining the crane and pit skimmers.
