On or about June 27th, 2024, employees engaged in roofing operations located at 4506 North 212th Street, Elkhorn, Nebraska 68022 were exposed to fall hazards in that employees were observed installing shingles on a steep pitched roof approximately 11 feet 9 inch ground to eave fall height without fall protection.
