29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  Employees engaged in residential roofing operations on the jobsite located at 20915 Hartman Ave, Elkhorn, Nebraska 68022, were exposed to eye struck-by hazards in that employees were observed using pneumatic nail guns without eye protection.  This was most recently documented on or about June 27th, 2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Employees engaged in residential roofing operations on the jobsite located 20915 Hartman Ave, Elkhorn, Nebraska 68022, were exposed to fall hazards in that multiple employees were observed working at heights greater than 6 feet and as high as 11 feet without any form of fall protection.  This was most recently documented on or about June 27th, 2024.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees engaged in residential roofing operations on the jobsite located at 20915 Hartman Ave, Elkhorn, Nebraska 68022, the employer failed to provide adequate fall protection training to his employees prior to the employee performing work that exposed them to fall hazards.  This was most recently documented on or about June 27th, 2024.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees engaged in residential roofing operations on the jobsite located at 20915 Hartman Ave, Elkhorn, Nebraska 68022 were exposed to fall hazards in that employees were using a multi-purpose aluminum ladder for access and egress to and from the roof, and the ladder did not extend at least 3-feet above the surface being accessed nor did the employer provide a handhold to assist with employees transition on and off the ladder. This was most recently documented on or about June 27th, 2024.
