29 CFR  1910.37(b)(4):  Where the direction of travel to the exit or exit discharge was not immediately apparent, signs were required to be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge.  Additionally, the line-of-sight to an exit sign was not clearly be visible at all times:  Warehouse - The direction of travel throughout the warehouse (approximately 52,000 square feet) was not equipped with exit signs directing employees to the nearest exit and exit discharge.

As of June 5, 2024, the employer had not provided training or certified that each employee working at this facility had received a combination of formal instruction, practical training, and evaluation of the operator's performance of powered industrial trucks.

29 CFR  1910.30(a)(1):(a) Fall hazards. Before any employee was exposed to a fall hazard, the employer did not provide training for each employee who used personal fall protection systems or who was required to be trained as specified elsewhere in this subpart.  Employers must ensure employees were trained in the requirements of this paragraph on or before May 17, 2017:  Warehouse - Employees utilizing fall protection equipment on the powered industrial trucks were not provided with training for the fall protection equipment they were utilizing.
