29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 27, 2024, at the location of a worksite located at 1239 Linda Street, Rocky River, OH : employees were exposed to a fall hazard of approximately 20 feet while performing roofing activities on an residential home . Employees were using extension ladders to access the roof and the employer did not ensure that they extended at least 3 feet above the upper landing surface. Two ladders were used in this manner, both on the front and rear of the building.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 27, 2024, at the location of a worksite located at 1239 Linda Street, Rocky River, OH: The employer did not ensure that employees performing roofing activities on an 8/12 sloped roof were protected from fall hazards. Employees were exposed to fall hazards of 20 feet to the ground below while performing roofing activities. Garcia Home Solutions, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1566853, citation number 1, item number 1 and was affirmed as a final order on January 27, 2022, with respect to a workplace located at 2208 Fairdale Ave, Cleveland, OH 44109
