29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   (a) On or about June 25, 2024, at the job site located at 2731 E. Forrest Hill Ave, Oak Creek WI 53154, the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected. Hazards identified included, but were not limited to fall protection not installed and/or used properly, safety glasses, and ladders not extending three feet above the landing surface.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about June 25, 2024, at the job site located at 2731 E. Forrest Hill Ave, Oak Creek WI 53154, employees were performing roofing work with pneumatic nail guns without the use of eye protection, exposing them to possible danger of eye injury from flying objects.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about June 25, 2024, at the job site located at 2731 E. Forrest Hill Ave, Oak Creek, WI 53154, employees conducting roofing work on the residential roof with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 18 9/10 feet from roof to the ground below.

Forrest Hill Ave, Oak Creek, WI 53154, employees who were performing roofing work on a two story residential house did not have fall protection training to recognize fall hazards and to implement fall protection methods.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about June 25, 2024, at the job site located at 2731 E. Forrest Hill Ave, Oak Creek, WI 53154, employees were accessing/egressing existing residential roof from a ladder which did not extend three feet above the upper landing surface. This exposed employees to a fall hazard.
