29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about June 25, 2024, at the job site located at 2830 E. Emily Ave, Oak Creek, WI 53154, employees conducting tear off work on the front side of a residential roof with a 10:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 10 feet from roof to the ground below.  (b) On or about June 25, 2024, at the job site located at 2830 E. Emily Ave, Oak Creek, WI 53154, employees conducting shingling work on the back side of a residential roof with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 18 5/10 feet from roof to the ground below.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about June 25,   2024, at the jobsite located at 2830 E. Emily Ave, Oak Creek WI, employees who were performing roofing work on a two story residential house did not have fall protection training.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about June 25 2024, at the job site located at 2830 E. Emily Ave, Oak Creek WI, employees were accessing/egressing existing residential roof from a ladder which did not extend three feet above the upper landing surface. This exposed employees to a fall hazard.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)   (a) On or about June 25, 2024, the employer did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected. Hazards identified included, but were not limited to fall protection not installed and/or used properly, safety glasses, and ladders not extending three feet above the landing surface.
