29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)On or about March 12, 2024, the employer did not ensure that energy control procedures were developed, documented, and utilized prior to allowing employees to perform service and maintenance activities on the Federal Press 45 on Machine 472 (serial unreadable-Half Rev Punch Press).  Employees were exposed to crushing injuries while attempting to correct a malfunctioning pin with their hands in the danger zone of the press.  The machine was not de-energized and locked out to prevent the unexpected start-up during this servicing and maintenance activity.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
