29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 20, 2024, B.E.T. Trucking, Inc. DBA The Sagamore Companies located at 2001 Barlow Rd. Hudson, OH 44236, did not ensure machine specific procedures for the Cementech C60 Series were developed, documented and utilized prior to service and maintenance of the equipment took place. Drivers tasked with operating the Cementech C60 Series remove guarding in place to clean out the Mixer and/or remove any debris jammed within the auger between loads and/or at the end of shift, while powered on exposing the drivers to the hazardous area.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On or about June 20, 2024, B.E.T. Trucking, Inc. DBA The Sagamore Companies located at 2001 Barlow Rd. Hudson, OH 44236, did not ensure that drivers and maintenance tasked with conducting service and/or maintenance activities on the Cementech C60 Series received the benefits of authorized employee training. These activities include but not limited to cleaning, unjamming, maintaining and/or servicing the equipment.
