29 CFR  1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a) 1463 S. Williams St., Westmont, IL 60559 - On or about June 25th, 2024, employees were exposed to an approximate 9 foot fall while accessing the roof after climbing, and stepping off the ledge of a truck-bed. A stairway or ladder was not provided at this point of access.   This item was corrected during inspection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1463 S. Williams St., Westmont, IL 60559 - On or about June 25th, 2024, employees were exposed to an approximate 18 foot fall while using a portable ladder that did not extend three feet past the landing surface.  This item was corrected during inspection.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) 1463 S. Williams St., Westmont, IL 60559 - On or about June 25th, 2024, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail gun. Eye protection was not being utilized while employees engaged in this activity.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Rhino Exterior, Inc. dba DM General Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA Inspection number 1633035, Citation 1, Item number 1 and was affirmed as a final order on January 27th, 2023, with respect to a workplace located at 1436 Bridgedale Rd., Crystal Lake, IL 60012.  Rhino Exterior, Inc. dba DM General Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA Inspection number 1418428, Citation 1, Item number 1 and was affirmed as a final order on December 4th, 2019, with respect to a workplace located at 414 Blue Ridge Dr., Oswego, IL 60543.

The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.   a) 1463 S. Employees were exposed to fall hazards while performing roofing work and the employer did not maintain records of training.
