29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) 1000 Virginia International Gateway Blvd. Portsmouth VA 23703 Pier side on or about 05/21/ 2024, employees were using extension cords that were not in accordance with its listing and labeling in that a yellow 50-foot extension cord was plugged into a GFCI pigtail extension cord with another extension plugged into it to supply power for multiple equipment. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 1000 Virginia International Gateway Blvd, Portsmouth VA 23703 Pier side on or about 05/21/ 2024, employees were using a yellow 50-foot extension cord plugged into a three-way GFCI pigtail extension cord where the outer jacket was pulled away from the male attachment plug, causing strain on the internal connection, thereby creating a potential electrical shock hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
