29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) At or near 50 Route 4 East, Paramus NJ - 2nd Floor: Employee without any means of fall protection was exposed to fall hazards of approximately 17 feet while engaged in welding activities of pour stops, on or about 6/26/24.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At or near 50 Route 4 East, Paramus NJ - 2nd Floor: Employee engaged in welding activities was exposed to fall hazards of approximately 16 feet while utilizing an extension ladder that only extended approximately 6 inches above the landing, on or about 6/26/24.
