29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about July 1, 2024, the employer did not ensure that employees engaged in roofing activities on a low-sloped roof located at 253 3rd Street Lasalle, IL. 61301 were protected from falling. There was no fall protection observed. Employees were exposed to a fall of greater than 15' to concrete and blacktop surfaces below which could result in fractured or broken bones, life altering disabilities, or death. Abated during inspection.

General-duty citation text
29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about July 1, 2024, the employer did not ensure a training program was provided to employees that were exposed to fall hazards. Five employees and the Foreman working on a commercial roof were exposed to falls of greater than 6' with no fall protection or safety monitor in place and no training provided. Employees were exposed to fall related injuries which could result in irreversible injury in permanent disability or death. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
