29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  LOCATION: The jobsite at 218-220 Old Colony Rd. On or about July 2nd, 2024  The employer exposed their employees to eye injuries when the use of appropriate eye/face protection was not used while using a reciprocating saw.

29 CFR  1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  LOCATION: The jobsite at 218-220 Old Colony Rd. On or about July 2nd, 2024  The employer exposed workers on site to fall hazards when they were working at the edge of 2nd floor "balcony" without the correct use of Personal Fall Aarrest Systems, a guardrail, or any other means.

29 CFR  1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.   LOCATION: The jobsite at 218-220 Old Colony Rd. On or about July 2nd, 2024  The employer exposed their workers to fall injuries when a snap hook carabeener that was not a locking type was used as part of the anchorage system for a vertical lifeline.

29 CFR  1926.502(d)(8):Horizontal lifelines shall be designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two.  LOCATION: The jobsite at 218-220 Old Colony Rd. On or about July 2nd, 2024  Workers on this site were exposed to fall hazards when the employer allowed the vertical lifelines to be knotted around the stud framing inside the building they did not ensure that the lifelines in use were installed and used under the supervision of a qualified or competent person.
