29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a new residential worksite located at 8400 N Marston Ave KC MO -- employees engaged in roofing duties were handing materials to employees working above and were not wearing head protection. This exposed the employees to struck-by hazards in that materials, tools and / or debris could have fallen on their heads. This was documented on or about 7/2/2024.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at 8400 N Marston Ave KC MO -- employees engaged in roofing duties using hammers and other small hand tools were not wearing eye protection. This exposed the employees to getting debris in their eyes causing injury. This was documented on or about 7/2/2024.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a new residential worksite located at 8400 N Marston Ave KC MO -- Employees engaged in a roofing a small demo like building that will include a sign for the business though not for use with people were engaged in roofing duties. The employees were working from height of 8 feet on the south side and over 10 feet on the north side without any personal fall arrest system. This exposed the employees to fall hazards. This was documented on or about 7/2/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a new residential worksite located at 8400 N Marston Ave KC MO -- employees engaged in roofing duties were using an extension ladder that did not extend 3 feet above the landing surface. This exposed the employees to fall hazards. This was documented on or about 7/2/2024.
