Workers were exposed to the hazard of thermal burns from the hot condensate/steam when performing this temporary operation. (b) On or about June 27, 2024, in the Platformer unit (#9) at the Lion Oil Refinery in El Dorado, AR, the employer did not identify, evaluate and control potential hazards with a written operating procedure for a temporary operation involving the condensate wash tank (Metal drum #009-030), pump (#009-2060) and its connected piping and valves. Workers were exposed to the hazard of thermal burns from the hot condensate/steam when performing this temporary operation.
