On or about July 03, 2024, at the site : the employer did not comply with the requirements of Table 1 of the standard when an employee was observed working in a visible cloud of dust while using a STIHL TS 420 Concrete Cut-Off Saw, without a water suppression system to cut the sidewalk, and the employer had not conducted an exposure assessment to determine the employee's exposure.

On or about July 03, 2024, at the site: an employee was observed working in a visible cloud of dust while using a STIHL TS 420 Concrete Cut-Off Saw Cutquik , without a water suppression system, to cut the concrete piece , and the employee had not received training related to respirable crystalline silica hazards and the requirements of the standard.
