29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.  On or about July 5, 2024, and at times prior thereto, on the southwest side of the jobsite; employees picked up roofing debris from the ground without using protective helmets while working below employees who were repairing a roof.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:  On or about July 5, 2024, and at times prior thereto; on the southwest side of the jobsite; employees repaired a roof without adequate fall protection while working on the second floor of an assisted living facility.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about June 21, 2024, on the northeast side of the jobsite; employees utilized a fall protection system without being provided training on hazard recognition and procedures to avoid hazards while installing shingles to an existing structure.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: Note:  If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:  On or about July 5, 2024, and at times prior thereto, on the southwest side of the jobsite; employees utilized a job made trash box without a manufacturer's written approval of the specification while loading the trash box with debris from the roof of a two-story building.
