29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Long Bend Drive Lot #42 Carlisle, PA - On or about July 5, 2024, employees were exposed to overhead hazards while working in close proximity to a knuckle boom crane's suspended load. Protective helmets were not used to protect employees from struck by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gasses or vapors, or potentially injurious light radiation: (a) Long Bend Drive Lot #42 Carlisle, PA - On or about July 5, 2024, employees were using tools, including but not limited to pneumatic nail guns, without any means of eye protection, exposing them to struck by hazards from flying particles. Abatement certification required within 10 days after abatement date. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Long Bend Drive Lot #42 Carlisle, PA - On or about July 5, 2024, employees installed roof trusses onto a two-story residential duplex home at heights of approximately 20-feet above grade without utilizing any means of fall protection. b) Long Bend Drive Lot #42 Carlisle, PA - On or about July 5, 2024, employees installed roof trusses onto a two-story residential duplex home at heights of approximately 8-feet above the interior floor without utilizing any means of fall protection. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
