29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: (a) On or about July 2, 2024, the employer did not ensure exits routes were unobstructed. The exit route leading to the Northeast emergency exit located in the production area, adjacent to the 3"x 4" Downspout roll forming machine, was obstructed by a pallet containing used hydraulic fluids. This condition exposed employees to fire hazards. (b) On or about July 2, 2024, the employer did not ensure exits routes were unobstructed. The exit routes to the rear warehouse, opposite the loading docks, were obstructed. This condition exposed employees to fire hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about July 2, 2024, the employer did not ensure employees were protected from rotating parts while utilizing a decoiler (Jobsite, Inc., Model No. PD7), as part of the 3" x 4" Downspout roll forming production line. This condition exposed employees to caught-in hazards. (b) On or about July 2, 2024, the employer did not ensure employees were protected from rotating parts while utilizing a decoiler (Jobsite, Inc., Model No. PD7), as part of the 3" x 3" Downspout roll forming production line. This condition exposed employees to caught-in hazards. (c) On or about July 2, 2024, the employer did not ensure employees were protected from caught in hazards while operating a hydraulic tilt table (Verti-lift, Inc. Model No. HUE 90-40-3640), located in the production area adjacent to the 3" x 3" and 3" x 4" Downspout roll forming production lines.
