29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 28, 2024, at the eastside of the jobsite, an employee was on the roof without fall protection while performing painting work on the exterior of the building.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about June 28, 2024, on the eastside of a commercial building, the side rails of an extension ladder did not extend at least 3 feet above the roof and was used by employees to access the roof.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired. On or about June 28, 2024, on the eastside of a commercial building, an employee used an extension ladder that had a structural defect in that a ladder foot was missing. Employees used the extension ladder to access the roof to perform painting work.
