1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: State Rd Plymouth MA On July 9th, 2024, employees were exposed to cave in hazards while working in a trench over 5 feet deep where cave in protection was not utilized. The employer did not have a program to conduct and document frequent and regualr inspections of their worksites to ensure that workers were protected from cave-ins.
