29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) On or about July 3, 2024, at the above addressed job site, employees installing communications cable were permitted to work in proximity to an energized 480 volt buss bar and were not protected against electric shock. No abatement certification or documentation is required for this item.

29 CFR 1926.417(b): The equipment and circuits that are needing to be deenergized were not rendered inoperative and did not have tags attached to all points where such equipment or circuits can be energized. a) On or about July 3, 2024, employees installing communications cable were permitted to work on equipment that was not rendered inoperative and did not have tags attached to all points which could be energized. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
