This exposed the employees to struck by hazards and eye injuries.

(a) On or about July 9, 2024, at 921 Durango Drive, Hartford, WI 53027, employees installing a roof on a new residential home with an approximately 10:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 15 feet 10 inches from the roof eve to the ground below on the back (East) side of the home.
