OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards from powdered metal press deficiencies:   A.) Comtec Mfg. LLC, On or about 6/19/24 and times prior thereto: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Powdered Metal Sizing Press S1 had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: protective cover on the two hand controls able to be removed with simple hand force.    B.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Powdered Metal Sizing Press S1 had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: a selector switch on the press to change the mode of the machine.  The switch allows for all employees to change the mode of the machine as it is not under authorized employee/supervisory control.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
