OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards from powdered metal press deficiencies:   A.) Comtec Mfg. LLC, On or about 6/19/24 and times prior thereto: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Powdered Metal Sizing Press S1 had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: protective cover on the two hand controls able to be removed with simple hand force.    B.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Powdered Metal Sizing Press S1 had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: a selector switch on the press to change the mode of the machine.  The switch allows for all employees to change the mode of the machine as it is not under authorized employee/supervisory control.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    A.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: Employees were performing service and maintenance activities on equipment including, but not limited to S1 Sizing machine.  The magnitude of energy was not noted on the machine specific energy control procedure.    B.)Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: Employees were performing service and maintenance activities on equipment including, but not limited to S1 Sizing machine. The employers written energy control procedures did not include steps to notify affected employees of the application or removal of lockout devices   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(e)(3):Specific procedures and training that were developed, documented and incorporated into the employer's energy control program for removal of a lockout or tagout device under the direction of the employer when the authorized employee  A.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: employees periodically conducted service, maintenance, and repairs on equipment.  The employer had not developed a procedure for use during removal of a lockout device by someone other than the authorized employee who applied it  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

LLC, On or about 7/9/24 and times prior thereto: The recordable injury and/or illness entries on the OSHA 300 Log for the Calendar Year 2023 were not completed as required.  In Column F Recordable case 797037-1 that occurred on 6/19/24 was not descriptive, in that the body part affected, and/or the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  B.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: The recordable injury and/or illness entries on the OSHA 300 Log for the Calendar Year 2023 were not completed as required.  In Column F Recordable case 795645-1 that occurred on 6/12/24 was not descriptive, in that the body part affected, and/or the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  C.) Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: The recordable injury and/or illness entries on the OSHA 300 Log for the Calendar Year 2023 were not completed as required.  In Column F Recordable case 784346 that occurred on  4/4/24 was not descriptive, in that the body part affected, and/or the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.    NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

Comtec Mfg. LLC, On or about 7/9/24 and times prior thereto: The employer did not conduct and document a periodic inspection of their energy control procedure on an annual basis. Employees were performing service and maintenance activities on equipment including, but not limited to S1 Sizing machine.
