OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees (a) On or about July 10, 2024, employees were exposed to struck-by hazards in that industrial storage racks were visibly damaged with bent and twisted beams and columns, and damaged or missing anchor bolts in locations throughout the warehouse and production areas. Pallets of water were stored on multiple levels of the racking systems.

The shaft protruded approximately 3 1/8 inches from the conveyor system, with a diameter of apprimoximately1 1/8"1" in diameter", and was not guarded by a nonrotating cap or safety sleeve.
