29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  (a) On or about July 10, 2024, at 1830 Quintard Ave, Anniston, AL: the employer did not maintain an accident prevention program to comply with the regulations of this part, which included, but were not limited to, fall protection when exposed to but not limited to, fall hazards associated with commercial roofing installation/repair operations.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

(a) On or about July 10, 2024, at 1830 Quintard Ave, Anniston, AL: the employer exposed employees to fall hazards, in that they did not initiate and maintain frequent and regular inspections before performing such as but not limited to installations/repair work on a commercial building.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) On or about July 10, 2024, at 1830 Quintard Ave, Anniston, AL:  the employer exposed employees to struck-by hazards, in that head protection was not utilized when employees were working under/below the ladder when employees were working on the roof.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about July 10, 2024, at 1830 Quintard Ave, Anniston, AL: the employer exposed employees to fall hazards, in that fall protection trainings was not provided to employees while doing roofing installation/repairs at heights over 6 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
